Dear Parent /Guardian,
Welcome to Basha High School for the 2016-2017 school year!We have prepared a parent registration checklist for new students to the District. Please be prepared to present these items at the time of registration, as students will not be allowed to register without the following information.
PROOF OF RESIDENCE – One of the following must be submitted:
· Complete copy of current utility bill (Gas, Electric, Water) that displays parent's name and the physical service address. Disconnect notices will not be accepted.
· Title Papers/Purchase Contract
· Lease/Rental Agreement (must be on letterhead of rental company)
To comply with Arizona State Law the following immunizations are required for school attendance.Immunization Records must comply with the Arizona State Law. Students not meeting these requirements will be excluded from school. Requirements include:
1. (2) dose series of Measles, Mumps, Rubella (MMR)
2. (3) dose series of hepatitis B (Hep B) vaccine
3. TD (Tetanus) within last 10 years for grades 10-12., within last 5 years for grade 9
4. Varicella (shot or history of chicken pox)
5. Polio – 3-4 doses depending on age
6. Meningococcal (Menactra) 1 dose currently for 9th grade only
Document signed by physician, physicians' office, health provider, or former school record with signature will be acceptable. If you are not sure what shots are required, please contact your local health department.
BIRTH CERTIFICATE (STATE CERTIFIED) – All students must have a Birth Certificate on file. If the birth certificate is lacking upon day of registration a copy must be submitted within thirty (30) days. No student may participate in AIA activities without a birth certificate on record.TRANSCRIPTS, DISCIPLINE AND ATTENDANCE RECORDS – schedule cannot be created without a record of credits the student has accomplished in their high school career.
WITHDRAWAL PAPERS – Students need official withdrawal documents and transfer grades if enrolling during the school year. Failure to provide transfer grades may result in a loss of credit.
LEGAL GUARDIANSHIP – Student must be registered by parent or legal guardian and attend school where the parent/legal guardian resides.
SPECIAL EDUCATION STUDENTS – CURRENT COPY of IEP, Psychological Evaluation, MET forms needed. Original evaluation, if available.
All forms contained in the Registration Packet must be completed and turned into the Registrar and include the following documents: StudentRegistration Form, Student EmergencyHealth & Medical History, CUSD FamilyCensus, Citizen Contract, Student History, StudentInformation Slip, EnglishAcquisition Services (PHLOTE) Form, CUSD BYOT, Responsible Use Form, InitialIdentification of Family Status Form, andInfinite CampusAccess Form.
All transportation guidelines, including FAQs and contact information, can be found on the district website (click here). All students that qualify for bussing, whether you choose to ride the bus or not, will receive their transportation letter during the week before school starts. Please be prepared to present these items. If the registrar's office is asked to request that the information be faxed from the previous school, your registration process may be delayed.
Copies of student's report cards, attendance , transcripts and test scores are helpful when setting up the student's schedule.
